Dear Sisters and Brothers in Christ Jesus:
The Small Membership Congregation Team (SMCT) is delighted to solicit nominations of small membership churches (fewer than 125 persons in average weekly attendance at its primary worship service) involved in outreach/mission programs, that engender new disciples of Jesus Christ and are making a significant social/spiritual impact in their local community/area.

We request that District Superintendents encourage any and all churches that, in their opinion, are exemplary in this regard over the past year to complete the attached application.

Upon completion, the application should be signed by you and forwarded to the Committee, no later than February 17th, 2012, by either U.S. Postal Service or email, to the attention of:
