Step 1:
Download Excel worksheet with weekly totals (updated 2/1/13)
Download Excel worksheet with monthly total (updated 2/1/13)
Step 2:
Each month, complete the worksheet on your computer and then attach it in an e-mail it to us.
NOTE: This form should be returned with your payment by the 10th of the month following receipt of the funds by the local church.
