Open Enrollment for Holston Conference health insurance plans will run from Oct. 15 through Nov. 15, 2019..
The information, enrollment, and change forms are available at http://holston.org/healthinsurance. Be sure to download the forms in order to make them fillable.
Please note: If you have no changes to your health insurance plan, and you do NOT have a flexible spending account, you do not have to do anything. Your insurance will continue as is under the new 2020 rates: Holston.org/2020ratesheet.
If you want to participate in a Flexible Spending Account, you MUST re-enroll every year. Enrollment forms can be found at http://holston.org/healthinsurance.
If you want to make changes to your plan or enroll for the first time, please complete the correct forms at http://holston.org/healthinsurance and email them to openenrollment@holston.org, or mail them to Holston Conference, Attn: Open Enrollment, P.O. Box 850, Alcoa, TN, 37701. No faxes, please!
If you need a hard copy of the forms mailed to you, please email openenrollment@holston.org and include your mailing address along with your request.
All forms must be received by the benefits office no later than Nov. 15, 2019.
