Need to cancel or waive your insurance coverage?

Download this Cancellation Form to drop coverage for a family member or cancel your health insurance. Remember, all clergy who are at least ¾ time are required to have health insurance.  Use this waiver form, if you are not currently enrolled in the Holston Plan and need to waive insurance coverage from Holston.

Need to Enroll?

If you do not currently participate in the Holston Conference Health Insurance Plan, are eligible, and would like to enroll in 2021, please complete the form below and email it to openenrollment@holston.org or mail it to Holston Annual Conference, Attn: Open Enrollment, P.O. Box 850, Alcoa, TN, 37701 before November 15, 2020.  No faxes, please.

Flexible Spending Account
If you would like to participate in the Flexible Spending Account or Dependent Care Spending Account through Progressive Benefits Solutions, complete the form below and return it with your enrollment form. You will access your funds with a debit card from PBS ("Benny Card") to use for medical or dependent care expenses. The FSA Maximum Contribution for 2021 is $2,750, and the DCAP Maximum Contribution for 2021 is $5,000.

Special Instructions for HDHP/HSA Plan

To learn more about the benefits and features of a High Deductible Health Plan with a Health Savings Account (HDHP/HSA), visit Health Equity.

If you choose to enroll in the HDHP/HSA plan, you must open a Health Savings Account with either Health Equity or the Holston Methodist Federal Credit Union.  If you choose Health Equity, the conference benefits office will open the account for you, and you will receive information via email from Heath Equity. If you choose HMFCU, then you will need to set up your account with them and notify the conference benefits office of your account number. 

The Holston Annual Conference contributes $600 for individual coverage or $1,200 for "employee +1" or family coverage to your Health Savings Account in January, and you arrange with your church/employer to have money automatically deposited into the account each month from your paycheck.  You will receive a payment card linked to your Health Savings Account that you will use when you incur medical expenses.


Already Enrolled?

No Changes?
If you are currently enrolled in a Holston Conference Health Insurance Plan, and you do NOT want to change anything AND you do NOT want to participate in the Flexible Spending Account program, you do not have to do anything. Your insurance coverage will remain the same.  Please note that the deductibles have changed for both the PPO and HDHP/HSA plans. In addition, some additional benefits have changed in the PPO plan. Please click here to see what has changed in 2021.  Checkout the 2021 rate sheet for your monthly premium amounts. 

No Changes But Need to Renew FSA?

If you have no changes to your plan, but you would like to renew your FSA, please complete this FSA Enrollment form and return it via email at openenrollment@holston.org or by mailing it to Holston Conference, ATTN: Open Enrollment, P.O. Box 850, Alcoa, TN., 37701. 

By law, FSA enrollments must be renewed every year. Forms are due before November 15, 2020. No faxes, please.
