Need to cancel or waive your insurance coverage?

Please complete this form, if you need to drop coverage for a family member or cancel your health insurance. Remember, all clergy who are at least ¾ time are required to have health insurance.  Use this waiver form, if you are not currently enrolled and need to waive insurance coverage from Holston.

Changes for 2020

The deductible and out-of-pocket amounts for the PPO plan remain the same as last year (see PPO and HDHP-HSA Benefit Comparison) for details.  The HDHP/HSA plan will have an increase in the deductible amount to $2,800 and $5,600 to remain compliant with federal regulations. Due to ever-increasing costs of health care, our premiums increased approximately 4% compared to last year (see 2020 rates). Dental and vision coverage can be requested on both the medical enrollment and change forms.

Your coverage and new premium amounts go into effect January 1, 2020.

How to Enroll for 2020 Health Care Plans

New Enrollees
If you do not currently participate in the Holston Conference Health Insurance Plan, are eligible, and would like to enroll in 2020, please complete the form below and email it to openenrollment@holston.org or mail it to Holston Annual Conference, Attn: Open Enrollment, P.O. Box 850, Alcoa, TN, 37701 before November 15, 2019.

Flexible Spending Account

Special Instructions for HDHP/HSA Plan
If you choose to enroll in the HDHP/HSA plan, you must open a Health Savings Account with either Health Equity or the Holston Methodist Federal Credit Union.  If you choose Health Equity, the conference benefits office will open the account for you, and you will receive information via email from Heath Equity. If you choose HMFCU, then you will need to set up your account with them and notify the conference benefits office of your account number. 

The Holston Annual Conference contributes $600 for individual coverage or $1,200 for family coverage to your Health Savings Account in January, and you arrange with your church/employer to have money automatically deposited into the account each month from your paycheck.  You will receive a payment card linked to your Health Savings Account that you will use when you incur medical expenses.

Current Participants

No Changes?
If you are currently enrolled in a Holston Conference Health Insurance Plan, and you do NOT want to change anything AND you do NOT want to participate in the Flexible Spending Account program, you do not have to do anything. Your insurance coverage will remain the same.  Please note that the Health Savings Account deductible for 2020 will be $2,800 and $5,600 to remain compliant with federal regulations. Checkout the 2020 rate sheet for your monthly premium amounts.

No Changes But Need to Renew FSA?

If you have no changes to your plan, but you would like to renew your FSA, please complete this form and return it via email at openenrollment@holston.org or by mailing it to Holston Conference, ATTN: Open Enrollment, P.O. Box 850, Alcoa, TN., 37701. 

By law, FSA enrollments must be renewed every year. Forms are due before November 15, 2019. No faxes, please.
